7/18/2023 EnchantaVaughn Approved (AM) Upon arrival, client is observed displaying appropriate behaviors when entering the camp building. Client said good morning to staff. Client was seen grabbing breakfast and using his manners by saying please and thank you. Client put away his belongings and asked for help when he needed assitance. Client was witnessed having a wonderful morning, as evidence by his cooperation in camp and positive interaction with others. Client was observed listening and following directions. Client was able to transition to pm activities and lunch in a civil manner, walking in line with his peers without pushing and shoving. (PM) Client joined his peers at lunch and engaged in age appropriate conversation. Client is observed sitting in his seat and cleaning up after himself. Client walks in line quietly when transitioning from setting to setting. Client returns to class in a good mood, as evidenced by him smiling and laughing. Client is monitored being a good helper and participating in group activity. Client displayed good sportsmanship at recess and was reported being very social, interacting well with peers. Client participated in psycho educational activity but had difficulty transitioning from his break to the session. (EOD) Client was reminded of his goals and objectives. Client joined his peers in putting away the board games and prepared for dismissal. Client was observed complying to rules and regulations. Client communicated that he was ready to go home and relax. Staff reported client expresses good thoughts and did not have a problem with him today. Client met objectives 1, 2, and 3, as witnessed. (AM) QMHP utilized active listening skills as client expressed himself throughout the day. QMHP kept client involved in activities that involved teamwork such as but not limited to team sports in efforts to observe his willingness to utilize the positive social skills he'd been learning. QMHP informed client of his positive behaviors. (PM) QMHP reinforced the importance of positive social communication between client and figures of authority. QMHP modeled positive prosocial behaviors for client in efforts to reinforce his positive interactions with his peers and the mental health professionals. (EOD) QMHP monitored client’s non-verbal communication with peers in efforts to encourage the use of encouraging communication cues versus discouraging cues. QMHP commended client on having a day with no redirection from anyone. QMHP encouraged client of his abilities to behave himself and be respectful to others. (AM) Client remained soft spoken as he interacted with QMHP and mental health professionals. Client participated in rapport building activity with QMHP. Client stated he was glad to be in camp. Client was able to interact with peers in an age-appropriate manner as evidenced by the client assisting those bullied. Client was observed displaying positive social interactions with his peers and staff. (PM) Client told QMHP that he did not have awareness of his negative impact on his peers and friends through his negative interactions with them. Client expressed that he would walk away when he is angered by a peer instead of lashing out with a negative response. (EOD) Client played appropriately with peers as evidenced by 0 complaints of negative behaviors made by peers or mental health professionals. Client maintained appropriate behaviors. Client displayed body language associated with joy and happiness around his peers. Instructor reported client was respectful and nice and didn't have to redirect client for anything. Client was receptive. Interventions were viewed as successful.
